Fractures and FissuresWhite House and congressional leaders continue to work feverishly on a sweeping overhaul of one-sixth of our economy, but their efforts hit a number of road blocks this week, putting their rapid timetable and even passage of major health reform legislation in jeopardy. The week started with White House Chief of Staff Rahm Emanuel signaling that President Obama would be willing to negotiate over creation of a government-run health insurance plan. Mr. Emanuel said one of several ways to meet the president's goals would be to have a public plan as a fallback if the marketplace fails to provide sufficient competition on its own.January 30, 2009
Stealth ReformCongressional leaders are arguing over whether they'll get a comprehensive health reform bill passed this year or next. But, in fact, major health reform is speeding through Congress in two bills that are on the fast track to enactment -- SCHIP and the economic stimulus bill. Expansion of the State Children's Health Insurance Program to children in families well into middle-income ranges passed the Senate yesterday and will likely be signed into law by President Obama early next week.

Compare and DecidePolitical and policy leaders in the U.S. see a new system of “comparative effectiveness reviews” as a solution to many of our problems in the health sector. This is coming, so we need to pay attention. We invited a leading European authority to conduct a seminar yesterday for our colleagues in the policy community to give us an overview. The idea is that a government-commissioned entity would “compare” various medical and pharmaceutical treatments to determine which are “effective” and make “recommendations” for care.
